COPD Outreach
About us
Chronic Obstructive Pulmonary Disease (COPD) Outreach provides a "Hospital at Home" (HaH) service for patients who fit the criteria and who present to the Emergency Department with an exacerbation of COPD. The service was set up under the guidance of Professor Richard Costello.
Performance Expectations
- To provide a high quality, patient friendly alternative to hospital care in the form of a "Hospital at Home" (HaH) programme.
- To provide a safe, planned early discharge of patients with an acute exacerbation of COPD.
- To alleviate the bed crisis by reducing hospital length of stay and admission rates.
- To provide an evidence-based pulmonary rehabilitation programme for stable COPD patients.
- To identify patients early and educate them and their families on self-management.
- To provide patients with the skills to maintain optimal health and provide a "Respiratory Passport" with an "Action Plan" to minimize the effect or even prevent the onset of an exacerbation.

Hours
Monday to Friday, 8.00am to 6.00pm.
Staff
Two full-time staff members provide extended hours of practice.

Awards
- In October 2009, Niamh McCormack won the Irish Medical Times Healthcare Award in the category of "Nursing Projects" for An Assessment of the impact of a respiratory passport for patients with COPD.
- In 2003, Ms. Claire Byrne (CNM II) was awarded the Beaumont Hospital Charitable Trust Nursing Research Award in recognition of her achievement in developing the project. The ERHA awarded the Inaugural Derek Dockery Award for Innovation in Health Care to the Programme.
