How to make a request under Routine Access
Your own personal medical records can be obtained through Routine (Direct) Access if you do not wish to request them under the FOI Act or Data Protection Act.
Your request can be made using the Routine (Direct) Access form (Link here) or via letter which can be sent to the address or email below:
It is important to provide the following information to us in order for us to assist you with your request
- Name, address (current & previous addresses if relevant), date of birth and signature
- Exact records you are requesting and for what date/period.
- Your contact telephone number.
- I.D for verification (copy passport or driver’s licence).
