How to make a request under the FOI Act of 2014

A standard request form for each of the 4 possible Freedom of Information (FOI) Request types is included in the guide.

1. Personal Information, including personal information of a minor or incapacitated Patient.
2. Personal Information of a deceased person (from next of kin or executor)
3. Non Personal Information can be requested by letter, e-mail or fax

If you are unable to find the information you are looking for, the following section provides information on how to make a Freedom of Information (FOI) request.

How to make an FOI request

Your application must be in writing stating that the information is being sought under the Freedom of Information (FOI) Act, 2014. You may submit your request by using the standard FOI Application Form, by writing to us or by email to foi@beaumont.ie. 

It is important to provide  as much information as possible in your application, as this will assist Beaumont Hospital  in dealing with your request.

When seeking your  own personal information, you will be required to prove your identity and will be requested to provide a copy of your Driving Licence, Passport or other form of identification. Please include a copy of this identification with any personal information FOI requests.

If you are a Next of Kin, and wish to request a copy of personal information on behalf of a deceased relative will need to complete this FORM

If you are requesting a copy of personal information for a person who is incapacitated, or a minor you will need to complete this FORM

You will also be required to provide other documentation s to verify your identity and your relationship with the person you are requesting the information on.  The above forms will give you more details.

Rights of Appeal

Under the Act, the person who has requested the information has the right to appeal an FOI body's decision - firstly to a more senior staff member within the FOI body than the staff member who made the original decision (referred to as an Internal Reviewer), and then to the Office of the Information Commissioner if the decision is still not satisfactory. The Information Commissioner's decision in this instance is binding.  See information under the Legislation Link
