Please download the Inter Hospital Transfer Form which should be fully completed and returned after phone discussion with the Urology registrar on-call (please phone our main hospital number @ 01 8093000 and ask to speak to the Urology Reg on call or alternatively call 01 797 8701) .
Incomplete forms will not be accepted.
