Going Home (Discharge)

You will generally be advised of your discharge the day before.   On the day of discharge your bed may be required for another patient and you may be transferred to our Transit Care / Discharge Unit while you wait for your discharge.  Instructions/letters or prescriptions will either be given to you by your doctor either in the ward or in the Discharge UInit

• Any valuables being kept for safe keeping will be returned to you.
• A medical certificate will be issued to you, if required.
• You will receive prescriptions for any medication your doctors have prescribed for you on discharge.
• Any follow up care will be organised and planned in accordance with your needs.
