FITT has been a core element of the management of older adults who present to Beaumont Hospitals’ Emergency Department (ED) since 2015. The Frailty Intervention Therapy Team aim to triage every older patient who presents to ED and work between 8am and 6pm. This inter-disciplinary screening process is completed within 1 hour of presentation, unless contraindicated medically. This facilitates timely FITT assessment in ED and initiation of the appropriate treatment pathway, whether a person requires admission or can be discharged home.
A multi-disciplinary approach is undertaken to address the complex issues associated with frailty. The team consists of the following professions:

We understand an Emergency Department attendance can be a challenging time for older people and their families. Our team, and the traditional ED nursing and medical teams, are here to support you as much as we can during your stay in ED and where appropriate after you leave.
Every patient aged 75 years or older who presents to ED is screened by the FIT team. Any patient aged 65 or older may be referred to the FIT team by ED medical or nursing team as appropriate.
