Care of the Older Person

Physiotherapists working in Care of the Elderly services provide assessment and treatment to patients in a range of different settings. 


Frailty Intervention Therapy Team 

Frailty is a major theme throughout all our work. We identify improved and different care models to enable the early discharge of our frail older patients. Our multidisciplinary team works in the Emergency Department (ED) and we aim to assess all patients over 75 years of age who present to the ED for frailty, initiate therapy assessments as needed and begin discharge planning. 


Acute Wards 

We have two 35 bedded specialist geriatric wards and additional medical and surgical wards which provide multidisciplinary rehabilitation and complex discharge planning for older adults. We provide a service to a wide range of conditions, consisting mainly of respiratory patients and frail older patients. 

We also cover such specialities as:

  • Cardiology
  • Post-operative care
  • ITU rehabilitation
  • Post amputee and vascular surgery 

This service has close links with our occupational therapy department to allow for a more fluid and efficient approach to patient care. 


St Joseph's Rehabilitation Unit 

There is a 20 bed rehabilitation ward in St Joseph's Hospital which provides expert MDT assessment and treatment. 


Day Hospital

For our outpatients, we have a day hospital service on the St. Joseph’s Hospital campus. The service provides comprehensive geriatric assessments for new patients to the consultant clinics, and review of our return patients to the clinic. Additionally, our consultants refer patients for multidisciplinary assessments and rehabilitation from their outpatient clinics. 


Raheny Community Nursing Unit 

On St. Joseph’s Hospital campus there is a 100 bed residential community nursing unit. We aim to provide assessment to new patients, routine manual handling assessments and rehabilitation for patients as needed.
