Where can I get more information about data protection and the hospital?

There are times when we need to share your information to ensure you receive the best care and treatment. We may share your information with your GP, Community services, other hospitals or care facilities.  We also give reports to organisations such as the HSE, HIQA, the National Cancer Programme, the National Office of Clinical Audit and others. These reports may be put together using information collected from patients.

How do I obtain a death notification form / death cert for a relative who has passed away in the hospital?

The Death Certs Office in the hospital will contact the next of kin of the deceased and will provide a Death Notification Form which can be collected at main reception or can be sent by registered post. You will need to bring this form to any Civil Registration Office to register the death and they will provide you with a Death Cert.

How do I get a copy of my medical records?

You have a right to access copies of medical records for yourself, another individual, or an organization.  Any such request will be processed under the Freedom of Information Act in order to protect your rights.  This can be arranged for you by contacting the Freedom of Information (FOI) Department on foi@beaumont.ie.
